Form D11 — the general application form within family proceedings

REVIEWED BY: Sean McNamara on 2026-09-01

What form D11 is

Form D11 is the HMCTS "application notice" — the general-purpose form for asking a family court to do something within proceedings that already exist. Whenever you need an order or a decision in a live case and there is no dedicated form for it, D11 is usually the form you use.

Official form page on GOV.UK: https://www.gov.uk/government/publications/apply-for-an-interim-order-as-part-of-divorce-dissolution-or-separation-court-proceedings-form-d11

What people use D11 for

  • Interim orders in divorce, dissolution or financial remedy proceedings (for example, interim maintenance).
  • Asking the court to excuse a procedural step or extend time.
  • Applications to vary or set aside a previous direction.
  • Asking for permission to rely on further evidence, or for directions about disclosure.
  • Any other request in existing proceedings that has no form of its own.

It is not the form for starting a new case: a C100 starts children proceedings, a D8 starts a divorce, and a Form A starts financial remedy proceedings. D11 works inside a case that has already been issued.

How to complete it

  • Case number: quote the existing case number exactly as it appears on court documents.
  • What you are applying for: state the order you want in plain, specific words — "an order that…" — not a narrative.
  • Why: set out briefly the information you want the court to take into account. If you rely on a witness statement, tick the relevant box and attach the statement to the application notice.
  • Statement of truth: the form must be signed and dated. The form warns that proceedings for contempt of court can be brought against anyone who signs a statement of truth without an honest belief in its truth — sign only what you believe is true.
  • Service: a copy normally has to be sent to the other parties. Check the Family Procedure Rules 2010 for the service requirements in your type of application: https://www.justice.gov.uk/courts/procedure-rules/family

Practical points for litigants in person

  • Keep it short and specific. Judges read dozens of these. One page of "what I want and why" beats ten pages of history.
  • Confidential addresses: as with other family forms, use the confidential contact details (C8) approach if your address must not be disclosed — the current form directs you to use the confidential details form rather than writing the address on the D11.
  • Fee: some D11 applications attract a court fee, others are free depending on the type of application. Check current fees and help-with-fees eligibility: https://www.gov.uk/get-help-with-court-fees
  • Where to file: the court office dealing with the existing case. Find the court via the official finder: https://www.gov.uk/find-court-tribunal
  • Always download the latest version of the form from the GOV.UK page rather than reusing an old copy.
